Instructional Testing Services

Test Request Form

Faculty planning to use Instructional Testing Services for their students, must complete this online form at the START OF EACH TERM.
Complete a separate form for each unique course. If you are teaching multiple sections of the same course, then you can complete one form -- list multiple sections and enter number of students accordingly.
