If you have received notification that an appeal is required, use the appropriate fillable form below. To determine if you are required to submit an appeal, check your myLane account under the myFinances tab; student requirements; and then the HOLD tab. Complete all forms in full, attach required documentation and sign. Incomplete, unsigned, or non-required forms will not be processed and will delay the processing of your aid.

Credit Limit Appeal – Complete this fillable form if you have been notified you may exceed the maximum credit limit. |

Satisfactory Academic Progress Appeal - If your financial aid status is "suspended", use this fillable form to explain circumstances that prevented you from meeting Satisfactory Academic Progress Standards. |
The following forms are used to request changes or updates to your financial aid. Please read form descriptions to determine if you are using the correct form and if they apply to your circumstances. Be sure to read all form instructions and select the correct academic year form before submitting. If you are unsure you are submitting the appropriate form, please contact Financial Aid for assistance.

2016/2017 Child Care/Dependent Care Budget Request - This fillable form is required if you are requesting that your cost of attendance be increased due to child (under age 13) or dependent care expenses. |
Foster Youth Volunteer hours log - Oregon Foster Youth certified by DHS may use this form to report their volunteer hours. |
2016/2017 Pell Adjustment Request - Use this form to request a Pell Grant adjustment if an institutional error prevented enrollment in the correct number of credits at the time a Pell Grant was calculated for the term. |
Preparatory Loan Funding Request - Students completing prerequisites to apply for admission to limited enrollment programs may use this form to be considered for preparatory loan funding. An information sheet is attached to the request to help you determine if this is the right option for you. |
Refusal to Provide Information and Support - Parent(s) may complete this form if they are refusing to provide required FAFSA information. |
Request For Change – Use this form for miscellaneous requests to Financial Aid, such as enrollment changes, cost of attendance changes, or accepting previously declined loans. |
Request To Apply For Financial Aid Without Parent Information – Dependent students may use this form if requesting to be considered an Independent student. See Dependent vs. Independent to determine if you may be eligible. |
Request to Increase Budget for Computer - Complete this form if you would like to increase your student budget for the purchase of a computer. |

2016-2017 Unsubsidized Stafford Loan Request - Complete this form to request an offer of the Federal Direct Unsubsidized Stafford Loan. Submit ONLY if you have already been awarded for 2016-2017. |
